RTOG 3515
A PHASE III, RANDOMIZED, PLACEBO-CONTROLLED, DOUBLE-BLIND, MULTI-CENTER, INTERNATIONAL STUDY OF DURVALUMAB FOLLOWING STEREOTACTIC BODY RADIATION THERAPY (SBRT) FOR THE TREATMENT OF PATIENTS WITH UNRESECTED STAGEI/II, LYMPH-NODE NEGATIVE NON-SMALL CELL LUNG CANCER (PACIFIC-4/RTOG-3515)
Credentialing requirement
- Step 1: All participants are asked to complete the Facility Questionnaire.
- Step 2: Please fill out the credentialing status inquiry form.
- Step 3: All participants are required to irradiate OSLD as part of an annual output check. If you are not already participating in this program, please complete the RTOG new participants demographics form to request OSLD.
- Step 4: To complete the credentialing process, participants must irradiate IROC Houston's IMRT thorax phantom. Please fill in the RTOG Foundation phantom request form online. IMRT credentialing will allow institutions to use either IMRT or 3DCRT treatments.
- Step 5: All participants must complete IGRT credentialing for soft tissue alignment. Please also complete the IGRT questionnaire.

RANDOMIZED PHASE III TRIAL OF CISPLATIN-BASED CHEMORADIOTHERAPY (CRT) NIVOLUMAB (ANTI-PD-1) IN PATIENTS WITH INTERMEDIATE AND HIGH-RISK LOCAL-REGIONALLY ADVANCED HEAD AND NECK SQUAMOUS CELL CARCINOMA (WITH PHASE I LEAD IN)
Credentialing requirement
- This trial will utilize TRIAD for dosimetry digital treatment data submission. TRIAD is the American College of Radiology’s (ACR) image exchange application, and it is used by the RTOG. See here for information on installing TRIAD.
- Please fill out the credentialing status inquiry form.
- All participants are required to irradiate OSLD as part of an annual output check. If you are not already participating in this program, please complete the RTOG Foundation New Participants Demographics Form to request OSLD.
- All participants are asked to complete the Facility Questionnaire
- All participants must irradiate IROC Houston's IMRT H&N phantom. Please fill in the RTOG Foundation phantom request form online.
- All participants must complete IGRT credentialing for boney alignment. Instructions can be found here. Please also complete the IGRT Questionnaire.

RANDOMIZED, DOUBLE BLINDED PHASE III TRIAL OF CISPLATIN AND ETOPOSIDE PLUS THORACIC RADIATION THERAPY FOLLOWED BY NIVOLUMAB/PLACEBO FOR LOCALLY ADVANCED NON-SMALL CELL LUNG
