Scholarship applicants are not eligible for online registration. Please complete the forms below and return them by fax or mail.

Step 2. Print out a scholarship form and complete all areas. Forms (English)(Spanish)
Step 3. Mail or fax the completed registration and scholarship form to the address/fax number listed on the form.
You must complete all sections of the scholarship form to be considered for a scholarship.
