Application Information

Requirements

All module and workshop participants must be licensed health care professionals (i.e., R.N., N.P., P.A. or M.D.). Interested individuals must submit:

Note: Enrollment will not be guaranteed without a completed application and a $100 deposit or total fees. The deposit, although non-refundable, will be applied to the total price of the module.

Fee balances are due no later than one month prior to the start of the module. Enrollments are limited due to intensive clinical instruction provided each participant. Therefore, class space is assigned on a first-come, first-served basis.

Confirmations & Cancellations

Confirmation letters are mailed to all approved applicants. The entire registration fee, less the $100 deposit, is refundable if requested in writing no later than one month before the start of the course. Cancellations after the one-month deadline will not receive refunds.

Classroom Location & Accommodations

Classes are held in room FC2.2047 (second floor of the Faculty Center, 1400 Holcombe Blvd.).

Hotels

Several area hotels offer a Texas Medical Center rate for our program participants and may also provide shuttle service to and from M. D. Anderson. Hotel accommodations are the responsibility of each participant.

State Employees: Many hotels in the Texas Medical Center area will accept state rates upon request. Be sure to bring your state I.D. badge and tax exempt certificate.
