Eligibility Requirements/Application Procedures

Application is made directly to the specialty program. Approval of recommendations, submitted by the clinical training program coordinator, is at the discretion of the vice president for academic affairs. Questions concerning the requirements for and status of applications should be directed to the clinical training program coordinator of the program(s) to which application is made.

The institution requires the following documentation to process a recommendation for appointment:

  • Recommendation for Appointment form
  • Application form - Submit an original application form. Supply all information requested. If you submit the form electronically, you must provide a signed paper copy to complete the application process.
  • Three letters of recommendation - Letters must be dated no more than one year prior to the application date. Letters must reflect appointment at the appropriate academic level and must be from persons qualified to comment on your qualifications in a laboratory or research environment, as well as in a patient care setting.
  • Transcripts - Arrange for official transcripts to be sent directly to you in a sealed envelope containing official letterhead from each medical school attended. Transfer credits listed with the school of graduation may not be used in lieu of an official transcript. Send updated transcripts as additional course work is completed. Graduates of medical schools outside the U. S. or Canada must provide a notarized copy of a currently valid ECFMG certificate in lieu of transcripts.
  • Pre-Appointment Physician Statement for Faculty/Education Staff form - This form must be signed by your physician.
  • Medical license or permit - Submit a copy of a currently valid authorization to practice medicine.
  • Translations - Documents in a language other than English (including Latin) must be accompanied by a certified translation.
  • ECFMG certificate - Graduates of non-U. S. and non-Canadian medical schools must provide a copy of an ECFMG certificate, valid at the program start date.

Additional documentation may be required for program review, medical licensure/permit applications and/or visa applications.

Programs have specific requirements for applicants. At a minimum, applicants must meet the following requirements:

  • Must be eligible to secure and maintain either a Texas Medical License, an Institutional Permit or Basic Postgraduate Resident Permit from the Texas State Board of Medical Examiners
  • Must be eligible to secure and maintain an appropriate visa status, if applicable
  • Must have successfully completed prerequisite training for the program

Several of our programs participate in match programs. Please note that candidates accepted through a match process must meet the minimum requirements stated above to be approved for appointment.
